DOE's Guidelines & Format for Trip Reports
General Requirements
The post-travel trip report is required for foreign travel when salary and/or travel expenses are funded from DOE monies. The trip report should be submitted to our office within 25 days after the completion of your trip. Trip reports for DOE employees and contractors are to consist of the two parts described below.
Part 1 - Include the following:
- An extract of the foreign travel request form, with an abstract of the results of the trip
- No classified information
- Information in the format & order below:
- Title of report (i.e., "Travel to_____")
2. Report date (MM,DD,YYYY);
3. Date(s) of travel (MM/DD/YYYY)
4. Name of all traveler(s) on this trip report
5. Position/title of traveler(s)
6. Employing organization Ernest Orlando Lawrence Berkeley National Laboratory and organization code LB
7. Business address, including street, city, state and zip code; One Cyclotron Road, Berkeley, CA 94720
8. DOE trip number; this number can be found in your email approval notice
9. Travel destinations (include organization, facility, city, state/province, country)
10. Brief purpose of the trip (include the justification from the foreign travel request)
11. People contacted
12. Facilities visited
13. Abstract (such as major highlights, benefits of the travel, results of meetings)
Part 2 - Include the following:
- Provide a complete and thorough report and analysis of the trip.
- Include the results of any meetings or discussions with representatives of foreign government(s) and/or company(ies);
- When more than one traveler follows the same schedule, only one member of the party needs to report on each meeting or event that took place.
- Any information about an incident that happened to the traveler or others including crime; theft; pick pocketing; unusual events on the street, in the hotel; on the way to or from work; interactions with police or other authorities; or any other situation that was unexpected or made the traveler feel uncomfortable. As an alternative to mentioning such an incident in the trip report, the traveler can contact cioffice@lbl.gov or call 486-4988.
- When appropriate, also include observations, conclusions, speculations and information relating to the safety, health and security of future travelers.
Submission and Distribution:
- Check with your Business Manager to determine if your Division requires a review prior to submitting this report.
- Via email as a PDF or DOC attachment to EWalker@lbl.gov and https://travelhelp.lbl.gov,
- The Berkeley Lab Travel Office will do the required distribution to DOE.
